Confidentiality / Medical Records:  All medical records are confidential. If you need to obtain copies of your medical records, a release is necessary. Please use our designated release form. Please bring or fax us a completed copy of the release.

To protect your privacy and respond to your records requests quickly, we use a Release of Information (ROI) company called Bactes to fulfill all records requests. Bactes comes to our office weekly and responds to your requests for medical records. Their toll-free line 800-560-3800 is open to you to check the status of your request.

Please note that there is sometimes a fee associated with providing records.

Disability Forms, Other forms:  Disability and other forms require one week for processing and require a $25 fee for completion. If you have not had an office visit within the past 60 days, you will need to make an appointment to review the status of your disability or discuss the nature of form you need completed.
